What information is required for the Petition?
Answer: The name change petition must include: The petitioner's desire and intention to change his or her name; The reason for seeking the change of name; The petitioner's current residence; and, The petitioner's residence or residence for and during the five years before the time when the petition will be filed.
Is publication required?
Answer: yes. Notice of the petition must be published in two newspapers of general circulation in the county where the petitioner resides or a county contiguous to that county. One of the publications may be in the official paper for the publication of legal notices in the county.
Is there a background check as part of the process?
Answer: yes. Petitioners are required to provide fingerprints, as well as certifications to ensure that the name change is not sought for the purpose of avoiding a judgment or lien.
